Three Mandatory forms need to be completed at the beginning of each school year. They are emailed out towards the end of August or the beginning of September each school year.
Please check your email account that you have on file with the school for the three emails from notifications@permissionclick.com with a link to complete the three mandatory forms listed below. 
  • Media Parent Release Form
  • Off-Campus Walking Field Trip Form
  • SD33 Technology Acceptable Use Agreement Form

 

**Please note that the above mandatory forms are the only forms that will be emailed to you. All other permission forms for field trips, athletics, etc, will be in your SD33 QuickPay account. You will need to use your login credentials to log in to your SD33 QuickPay account to complete and pay for your child's field trips, athletics, and other fees.**
